You are eligible to apply for coverage to begin the first of any month with CDPHP and your rate will be good for one year from the effective date.  You have the option to select two different plans for your company.  This allows small businesses the same flexibility usually reserved for large companies.  Clicking on the icon below will provide you with a benefit outline for the available programs.  Since rates vary by region and group size, we ask that you contact our office at (800) 300-0621 for information relating to your specific area.



CDPHP

CDPHP Plans are available in 24 counties of eastern and central New York.  Those counties include:  Albany, Broome, Chenango, Columbia, Delaware, Dutchess, Essex, Fulton, Greene, Hamilton, Herkimer, Madison, Montgomery, Oneida, Orange, Otsego, Rensselaer, Saratoga, Schenectady, Schoharie, Tioga, Ulster, Warren and Washington.  These plans are very cost effective.  Plans offered for sole proprietors are the same as offered for groups with employees with a rate differential for sole proprietors.
