GHI plans are available in all 62 counties in New York with rates varying according to the county of your business location.  Plan designs and rates offered for groups with employees differ from those offered to sole proprietors.  Highlights include, no copay for dependent children and students for office or specialist visits whether sick or well, over one million
in-network providers nationally and $0 copay for generic medications.

CDPHP

CDPHP Plans are available in 24 counties of eastern and central New York.  Those counties include:  Albany, Broome, Chenango, Columbia, Delaware, Dutchess, Essex, Fulton, Greene, Hamilton, Herkimer, Madison, Montgomery, Oneida, Orange, Otsego, Rensselaer, Saratoga, Schenectady, Schoharie, Tioga, Ulster, Warren and Washington.  These plans are very cost effective.  Plans offered for sole proprietors are the same as offered for groups with employees with a rate differential for sole proprietors.

Underwritten by Emblem Health, benefits are the same in all 62 counties.

Participating Agency Program

Referral Agency Program
